By submitting this application to Sandy Paws Rescue's 'Lending a Paw', you agree to the following:
-I agree to not obtain any other pets that are not listed on this form and to not use my pets for breeding purposes.
-I agree to have my pets licensed in my current town within 1 Month of receiving assistance, and I will provide proof to Sandy Paws Rescue 'Lending a Paw'. Failure to do so will result in disqualification from Sandy Paws Rescue's 'Lending a Paw' program.
-I agree that the food that we receive from Sandy Paws Rescue's 'Lending a Paw' may not be what my pet is used, and may cause a reaction.
-I agree to not hold Sandy Paws Rescue, and any of their volunteers liable for any adverse reactions that my pet may have.
-I agree that Sandy Paws Rescue 'Lending a Paw' is here to provide temporary assistance, and not to abuse the program.
-I understand that I must reapply every 3 months.
-I understand that Sandy Paws Rescue's 'Lending a Paw' program will not provide me with any direct funding. I will ONLY receive pet products (food, bedding, etc) and/or vouchers for vaccination services at Sandy Paws Rescue's veterinarian partner(s).
