Sandy Paws Rescue Adoption Application
Adopting a dog is a major decision, too many dogs end up in rescues/shelters because people aren't prepared for a dog. We're serious, you need to be serious. Please read through this application before submitting as it will not be accepted without ALL fields filled out with the information required.
Some things we will need:
- Vet information. (If you don't currently have one, you will need one before adoption)
- Please be aware that vets in your local area may not be accepting new clients at this time. It is best to be prepared and call around prior to applying to adopt to find out who is accepting new clients if necessary.
- Personal references. (Please call them, let them know we will be calling)
- Name of trainer. (If you don't currently have one, you will need to research one you would use if needed)
We do our best to provide you with an accurate description of a dog's age, breed, health and temperament. HOWEVER we do not always have the full history on a dog and therefore we are unable to guarantee anything about the dog you rescue. Dogs and puppies pee, poop, chew, bark, need time and training.
SPR is an all volunteer organization, do you understand that by filling out this application means you are 100% serious in moving forward with the adoption process? Please only fill out this application if you are 100% serious in moving forward with SPR's adoption process.
Ready to adopt?
The adoption fee includes:

- Age-appropriate vaccinations (DHLPP/DA2PP: Distemper/Hepatitis /Parainfluenza/Parvo vaccine)
- Bordetella/kennel cough vaccine (only administered if the dog has been transported from out of state; local surrenders do not receive this vaccine)
- Rabies certificate for dogs 15 weeks and older
- Parasite prevention treatments (de-worming & fecal tests)
- Spaying/Neutering (If a dog is not altered upon arrival, SPR will discuss arrangements with the adopter)
- Heartworm testing for adult dogs
- Multiple wellness exam visits
- Health certificate required for transport
- heartworm preventative
- flea/tick topical preventative
- Transport to Massachusetts*
- 48-hour quarantine in a USDA-approved isolation facility*
- Recheck by a licensed veterinarian post quarantine*
- any necessary medical treatments needed before adoption(this may not include cosmetic procedures like cherry eye, mole removals, specific dental procedures, etc.)
